Job Description
| Care Management nurses work directly with patients out of an office, by phone and in person. We focus on finding creative ways to help them toward better health, doing things like connecting them with community groups, helping them to look at their health in a new and empowered way, and translating doctors' instructions into plain English. Our specialty is being the patient's partner and friend. This is in-person position. |
